You have been hired to conduct a risk management review of a small adventure tourism company. The organization, which offers part-day and multi-day sea kayaking trips off the coast of British Columbia and Washington state in the USA, has had a significant near-miss activity, where a kayaker was separated from the group, capsized, and had to be rescued by the Coast Guard.

You have been retained to assess the organization’s safety practices. As part of this project, you will analyze the incident where a kayaker went missing, and identify the causal factors in each risk domain.

In your small group, review the material about the incident. Then use that material to fill out the form at https://forms.exped.net/domains-worksheet, listing all the risk factors in each of the eight risk domains listed there. When the form has been filled out, click ‘Submit’ then ‘Download PDF’ to obtain your copy of the filled-out form.

The scope of your review is on the field portions of the organization, such as staffing, equipment, activities, and activity areas. Your scope does not focus on issues of transportation, business administration, and participants (including medical  screening), so you can expect there may be limited information about these risk domains.

The organization is supplying you with several documents for your review, including:

1. An incident report of the capsize and rescue

2. A summary of incidents in the organization’s history

3. The organization’s Risk Management Plan

4. Examples of nautical chart (map) and tides and currents information used by guides

You will be interviewing three individuals:

1. The outdoor program’s director, who is also the founder and owner of the organization

2. The participant who was directly involved in the near-miss incident

3. The organization’s safety advisor, who is part of the program’s informal Risk Management Committee

You may find it useful to divide the documents and interviews amongst your small group members to save time.
